About this role
Anglia Case Management (ACM) is a well-established specialist case management provider, supporting adults and children with complex disabilities and neurological conditions. Established in 1998, ACM provides case management, rehabilitation, expert witness and care and support services, working with clients and their families to develop individual solutions that help them maximise their potential.
We are looking for an experienced Case Manager to join ACM, covering a caseload across Norfolk and Suffolk. This is an excellent opportunity for someone who enjoys working independently, coordinating multidisciplinary teams and making a meaningful difference to the lives of clients and their families.
As a Case Manager, you will manage a caseload of clients with complex needs, working closely with them, their families and wider professional teams to assess rehabilitation needs, develop case management plans and coordinate appropriate services and support.

The Role
We offer flexible remote working, with full-time preferred but a minimum of three days per week considered. You’ll manage a caseload across Norfolk and Suffolk, so regular travel is required, with occasional office attendance for meetings and collaborative sessions.
Key Responsibilities
- Undertake detailed assessments and identify clients’ rehabilitation and support needs
- Develop and regularly review individualised case management plans
- Commission and coordinate appropriate rehabilitation services across the statutory and independent sectors
- Coordinate multidisciplinary teams and monitor the effectiveness of interventions
- Undertake risk assessments and develop care and support packages where required
- Liaise with clients, families, solicitors, Deputies, and other professionals
- Produce assessment and review reports and maintain accurate case records
- Support clients with housing, equipment, education, leisure, and community-based needs
- Work with educational establishments where appropriate, including involvement with EHCPs and transition planning
- Provide advice and support to clients, families, and care teams
- Manage and supervise care staff where required
About You
- Previous experience working as a Case Manager with complex clients
- A relevant professional qualification and current professional registration
- Experience of acquired brain injury, neurological conditions or other complex disabilities
- Experience working with children and/or young people would be desirable
- Strong assessment, report-writing and care planning skills
- Experience coordinating multidisciplinary teams and commissioning services
- Excellent communication and relationship-building skills
- The ability to work autonomously while being part of a wider clinical team
- A full UK driving licence
