About this role

Are you a passionate Clinical Leader who believes that everyone deserves the opportunity to live life in their own home, on their own terms?

Do you want to be part of a growing business where your expertise, ideas and leadership can make a genuine difference to people’s lives?

Growth means new opportunities, new challenges and more chances to make a difference. As we continue grow, we’re looking for an inspiring and driven Clinical Manager who shares our ambition to deliver the highest standards of care while keeping the individual at the heart of everything we do and help us shape the future of home based complex care!

This is your opportunity to bring your clinical expertise, leadership skills and passion for outstanding care to a role where no two days are the same one day you could be helping someone leave hospital safely and get back into their own home. The next, you could be supporting the team to stabilise a complex care package, preventing a hospital admission

As Clinical Care Manager you will take clinical ownership of care packages across your region, working closely with Care Managers, Care Co-ordinators, Support Workers, families and multidisciplinary teams to ensure people receive safe, high-quality and truly personalised care in the place they call home.
