About this role

We have an exciting opportunity for an experienced Case Manager to join our team.

We are seeking a qualified and experienced Case Manager with a professional background in Nursing, Occupational Therapy, Physiotherapy, Speech and Language Therapy, Psychology or Social Work, and a proven track record in case management.

CCMS is an award-winning case management service with a CQC Outstanding rating, specialising in the rehabilitation of adults and children following brain injury, spinal injury and other catastrophic injuries.

As a Case Manager, you will work closely with clients and their families to assess rehabilitation needs, commission services, and lead multidisciplinary teams. You will coordinate and oversee rehabilitation programmes, ensuring clients receive the highest quality support within their communities, enabling them to achieve their optimum level of independence and quality of life.

The Role:

Location: Yorkshire and the North East

Office Base: Darlington (Hybrid) – predominantly home-based, with 1–2 days per month in the office.

Working Hours: Our standard working pattern is Monday to Friday, 37.5 hours per week. However, we understand that flexibility is important. If you're looking for different working hours, we'd be happy to discuss this with you and will accommodate flexible working wherever possible, taking into account the needs of the service.

Key Responsibilities:

As an experienced clinician, you'll use your clinical skills and manage your own caseload of clients. The organisation supports clients in the North East and Yorkshire who have an acquired a brain injury, spinal injury or life-changing physical disabilities due to an accident or a birth injury.

  • When a referral is received, you'll be responsible for completing a thorough and detailed assessment of their needs. They take a holistic approach so this will include their care, emotional and social needs. You'll implement a rehabilitation package and continually review and assess it, to ensure that it continues to meet the client's needs.
  • Most clients are involved in a medico-legal process and you'll support their case through accurate report writing and continual re-assessment. You'll be able to put into place bespoke equipment, vehicles, holiday's etc. that will benefit the clients health and emotional needs.
  • You'll work within a Multi-disciplinary team consisting of nurses, OT's, Social Workers and Physiotherapists and be able to share knowledge and experiences with others in the office.

About You:

  • Previous Case Management experience is essential.
  • Professionally qualified as a Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, or Psychologist.
  • A minimum of 3 years’ experience within your field of expertise.
  • Full UK Driving License.
  • Structured and logical thinking style that is comfortable operating both at a detailed level and at a directional level.
  • Excellent report writing skills.
  • Highly effective verbal and written communication skills.
  • Builds therapeutic relationships quickly and have strong interpersonal skills with clients and professionals.
  • Works autonomously and also as part of a team.
  • Demonstrates excellent time management skills and also co-ordinate others when working to deadlines.
  • Open to learning and able to apply new knowledge effectively within the workplace.
  • IT literate and have the ability to learn new systems quickly.
  • Strong problem-solving skills and the ability to manage difficult and at times, volatile situations.
  • Successful candidates will be required to undergo an Enhanced DBS.
