Full UK Driving Licence Essential

Candidates can be based in any location across England or Wales, provided they are willing and able to travel nationally.

As a Rapid Response Support Worker, you’ll be part of a specialist team deployed where support is needed most. One week you could be helping someone with complex needs leave hospital and safely return home; the next, you could be stabilising a care package to prevent a hospital admission and enable someone to remain living independently.

You’ll step into critical situations, bring stability, confidence and compassion, and make a genuine difference when it matters most.

Travel is a core part of this role: This is a national role and regular travel across England and Wales is required.

You must be comfortable:

  • Travelling to assignments across the UK
  • Working both live-in and live-out packages
  • Staying away from home when required

To support you:

  • Mileage is paid for business travel
  • Accommodation is provided when assignments are outside reasonable daily travelling distance
  • Accommodation is also provided for live-out packages where an overnight stay is required

You will support the set-up, stabilisation and ongoing delivery of complex care packages including:

  • Supporting safe and timely hospital discharges
  • Delivering high-quality personal care and daily living support
  • Establishing routines that promote independence and wellbeing
  • Providing reassurance and continuity to clients and families
  • Mentoring and supporting new Support Workers
  • Working closely with Care Managers and clinical professionals
  • Organising medication, supplies and essential care equipment
  • Identifying and escalating concerns appropriately


Flexibility That Works Both Ways:

  • Contracted to 162 hours per month
  • Planned breaks between assignments (depending on assignment length)
  • Overtime opportunities may be available
  • Live-in placements, sometimes lasting two weeks or longer for continuity of care
  • Live-out placements with accommodation provided where required
  • Opportunities to work across a variety of locations and care environments


What we are looking for:

Essential:

  • Full UK Driving Licence
  • Proven experience supporting individuals with complex care needs
  • Ability to work independently and adapt quickly to new environments
  • Excellent communication and organisational skills
  • Professional, calm and compassionate approach

Desirable:

  • Hospital discharge experience
  • Level 2 Health & Social Care qualification (or above)
