We'd like you to join us as a Rehabilitation assistant at our service based in London.

At Blackheath Brain Injury Rehabilitation Centre, we are specialists in the treatment of patients over the age of 18 with physical and cognitive impairments and/or neuropsychiatric disorders resulting from a brain injury or other neurological condition. We support our patients through assessment, rehabilitation, support and care across two wards.

What you'll be working:

Your hours are based on a rota working every other weekend.

• Full-time hours available
• 37.63 hours per week
• Candidates must be able to work day & night shifts
• Shift times (07:30-19:45) & (19:30-07:45)
• £27,960.51 (FTE) Per annum, pro rata

What you'll be doing:

• Providing a safe, caring, and supportive environment for adults with diverse needs, including mental health conditions, complex care requirements, or those undergoing rehabilitation.
• Supporting individuals with daily routines, including personal care, meal preparation, medication prompts, mobility, and household tasks, in line with their independence and preferences.
• Ensuring the safety, well-being, and dignity of residents by adhering to safeguarding policies and procedures, and reporting any concerns, incidents, or disclosures in accordance with safeguarding protocols.
• Implementing individualised care and support plans tailored to each person’s goals, needs, and outcomes, regularly contributing to reviews and updates.
• Maintaining accurate and timely records of care delivered, daily activities, incidents, and progress, using both written and electronic systems.
• Supporting individuals in managing behaviours that challenge through positive behaviour support strategies, de-escalation techniques, and promoting emotional regulation.
• Encouraging engagement in meaningful activities, therapeutic programmes, community outings, and social interactions that support mental, emotional, and physical well-being.
• Promoting independence, choice, and inclusion in all aspects of daily life, empowering individuals to participate in their care and decision-making.
• Collaborating effectively with colleagues, healthcare professionals, social workers, therapists, and external agencies to ensure consistent, high-quality support.

What you'll have:

• A genuine interest in providing person-centred care to adults with a range of needs, including those with mental health conditions, complex care requirements, and those in rehabilitation settings.
• Empathy, patience, and a respectful, non-judgemental approach when supporting vulnerable adults, always promoting dignity, independence, and choice.
• Experience with behaviour management and Positive Behaviour Support (PBS), including the use of de-escalation techniques in a care environment.
