About the Role
Due to continued growth, we are looking for an experienced and motivated Senior Case Manager to join our team in the Wigan area.
This is an exciting opportunity for an established Case Manager who is looking to take the next step in their career. Alongside managing a complex caseload, you will play a key role in the mentoring, supervision and development of other Case Managers, helping to build clinical excellence and consistency across the business.
You will be supported by the wider ACG Case Management Group, providing access to extensive professional development opportunities, clinical support, and a network of experienced practitioners. Drawing on responsibilities from the existing Senior Case Manager role profile, this position combines caseload management with leadership, professional support, and business development activities.
Key Responsibilities
Clinical Leadership
- Manage a caseload of clients with complex injuries and disabilities.
- Complete holistic assessments and develop outcome-focused rehabilitation plans.
- Coordinate and commission rehabilitation services to achieve the best possible outcomes.
- Ensure all case management activity is evidence-based, person-centred and compliant with professional standards.
- Maintain high-quality client documentation and reporting.
Mentoring & Management
- Provide mentoring, guidance and support to Case Managers across the business.
- Deliver regular supervision and support professional development.
- Support new Case Managers through induction and onboarding.
- Assist the Clinical Lead with performance management and annual appraisal processes.
- Promote best practice, consistency and high standards of case management delivery.
- Act as a senior resource for Case Managers requiring advice and support on complex cases.
Business Development
- Develop and maintain relationships with referrers, solicitors, insurers and other stakeholders.
- Represent the business at networking events and conferences.
- Support growth initiatives and identify opportunities for business development.
- Act as an ambassador for the business and the wider ACG Group.
Quality & Governance
- Support clinical governance and quality assurance activities.
- Contribute to audits, service improvement initiatives and compliance reviews.
- Ensure safeguarding responsibilities are fulfilled and best practice standards maintained.
- Promote a culture of continuous learning and professional excellence.
Leadership Support
- Work closely with the Clinical Lead and Senior Management Team.
- Provide clinical leadership support during periods of leave or absence.
- Contribute to strategic discussions regarding service development and workforce planning.
- Support recruitment and selection activities as required.
About You
Essential
- Registered healthcare professional (Nurse, Occupational Therapist, Physiotherapist, Social Worker or equivalent).
- Current professional registration.
- Minimum 2 years' Case Management experience.
- Experience supervising, mentoring or supporting other professionals.
- Strong understanding of rehabilitation and complex case management.
- Excellent written and verbal communication skills.
- Ability to build and maintain strong stakeholder relationships.
- Full UK driving licence and willingness to travel within the region.
Desirable
- Experience working with catastrophic injury and complex rehabilitation cases.
- Knowledge of CQC standards and regulatory requirements.
- Previous people management experience.
- BABICM membership and progression towards Advanced Practitioner competency.
